Job Description
Provides a comprehensive medical care to adult/pediatric patients in telemedicine settings, with a primary focus on diabetology within the scope of family medicine. Leads the diagnosis, treatment, and long-term management of diabetes and related metabolic disorders, alongside a broad range of acute and chronic medical conditions. Delivers evidence based, patient centered care through multidisciplinary collaboration, emphasizing prevention, early intervention, chronic disease optimization, and continuity of care. Applies population health and value-based care principles to improve clinical outcomes, enhance patient experience, and optimize healthcare resource utilization
Patient Consultations and Management
Provides a comprehensive medical care to adult/pediatric patients in telemedicine settings, with a primary focus on diabetology within the scope of family medicine. Leads the diagnosis, treatment, and long-term management of diabetes and related metabolic disorders, alongside a broad range of acute and chronic medical conditions. Delivers evidence based, patient centered care through multidisciplinary collaboration, emphasizing prevention, early intervention, chronic disease optimization, and continuity of care. Applies population health and value-based care principles to improve clinical outcomes, enhance patient experience, and optimize healthcare resource utilization
Patient Consultations and Management
- Deliver comprehensive telemedicine care for patients through virtual assessment, diagnosis, treatment, and follow-up of diabetes, related metabolic disorders, and other acute and chronic medical conditions.
- Conduct comprehensive clinical evaluations, including medical history, virtual physical assessment, medication review, diabetes risk assessment, and interpretation of relevant laboratory and diagnostic findings.
- Develop and implement individualized, evidence-based management plans, including glycemic optimization, medication management, lifestyle modification, preventive care, and specialist referrals when indicated.
- Manage patients with type 1 diabetes, type 2 diabetes, prediabetes, obesity, and other metabolic conditions, while preventing, identifying, and managing diabetes-related complications and associated comorbidities.
- Collaborate with multidisciplinary healthcare teams, including endocrinologists, dietitians, diabetes educators, pharmacists, and primary care providers, to deliver coordinated, patient-centered care.
- Utilize digital health technologies, including remote patient monitoring and telemedicine platforms, to provide safe, high-quality virtual care while maintaining accurate and timely clinical documentation.
- Promote preventive care, chronic disease management, population health, and value-based care initiatives to improve glycemic control, reduce diabetes-related complications, enhance patient outcomes, and optimize healthcare resource utilization.
- Communicate effectively with patients and their families, providing clear and concise explanations of diagnoses, treatment options, and follow-up care
- Adhere to all relevant medical regulations, ethical guidelines, and quality assurance standards for telehealth practice
- Maintain up to date knowledge of telehealth regulations and best practices. Participate in quality improvement initiatives and document patient encounters accurately
- Participate in quality improvement initiatives, research activities, and clinical audits to enhance patient care and outcomes
- Regularly review compliance protocols and participate in training sessions.
- Collaborate with compliance officers to ensure all practices meet regulatory standards.
- Prepare reports for audits and inspections, ensuring accuracy and completeness.
- Stay updated on changes in healthcare regulations and implement necessary adjustments
- Continuously enhance professional knowledge and skills in family medicine with a focus on diabetology, in-person and telehealth best practices
- Attend relevant conferences, workshops, and online courses.
- Stay up to date on the latest advancements in diabetology and telehealth technology
- Maintain accurate and complete patient records in accordance with HIPAA regulations
- Maintain proficiency in utilizing the designated telehealth platform, including video conferencing, secure messaging, and electronic health records (EHR) integration
- Actively participate in training programs and stay updated on platform enhancements and best practices. Collaborate with IT support to troubleshoot any technical issues
- Maintain compliance with all patient safety standards and ensure patient safety across all clinical interactions
- Report any patient safety risks or issues to the appropriate staff in quality department
- Practice safe medicine to ensure no patient harm
- Explain medications possible side effects and proper use
- Maintain satisfactory patient’s consultations through communication, taking complete history, providing empathy and understanding of patient’s conditions and circumstances, explanation of diagnosis and proposed management plan and answering patient queries
- Monitor patient feedback and address concerns promptly.
- Implement safety protocols to minimize risks during consultations.
- Conduct regular training sessions on patient safety for healthcare staff.
- Utilize patient satisfaction surveys to gather insights for continuous improvement.
- Collaborate with multidisciplinary teams to optimize patient care
- Share insights and recommendations to support patient-centered care approaches.
- Facilitate communication between team members regarding patient status and needs.
- Mentor junior staff and provide training on clinical protocols
- Medical degree (MBBS, MD or equivalent) from an accredited medical school.
- Residency training in Family Medicine from an accredited program.
- Board certification in Family Medicine
- Fellowship certification in Diabetology.
- Saudi Commission for Health Specialties SCFHS classification and registration as Family Medicine- Diabetology Consultant.
- Proven experience as a Family Medicine Diabetology Consultant or similar role, preferably in a hospital or clinical setting.
- Experience in practicing population health management and value-based care approaches.
- Proficiency in using electronic medical records (EMR) and telemedicine platforms.
- Strong clinical judgment and analytical skills.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with patients and colleagues.
- Ability to work effectively in a fast-paced environment and manage multiple priorities.
- Commitment to ongoing professional development and staying abreast of the latest advancements in family medicine with a focus on diabetology, population health management, and value-based care
