Job Description

Lead and oversee the end-to-end operations related to all partnership and referral activities. This encompasses managing clinical home visits, laboratory services, physical therapy, vaccinations, imaging, and other relevant components across various connected care channels and patient touchpoints. The role will be critical in ensuring the seamless management of these services, ultimately contributing to improved health outcomes for patients.

Additionally, building and managing the radiology, laboratory, and home health care departments within integrated care clinics, as well as overseeing their daily operations

Strategic Oversight

• Leads multidisciplinary healthcare operations across multiple care settings, ensuring integrated service delivery through partnerships, efficient processes, and data-driven improvements. Oversees home healthcare and diagnostic services to optimize performance, coordination, and patient satisfaction.

Lead quality care & exceptional customer service

• Drives continuous quality improvement by implementing standards, monitoring performance, and resolving service issues using KPIs and data insights. Enhances patient outcomes, satisfaction, and operational efficiency while ensuring compliance and staff development

Lead the relationship with internal and external stakeholders to manage service levels and improve health outcomes and safety

• Builds and manages strong stakeholder relationships to align goals, optimize care coordination, and ensure effective SLA performance. Promotes patient safety and regulatory compliance through evidence based practices, proactive risk reduction, and timely issue resolution

Lead Medical Quality Assurance & Regulatory Compliance

• Leads quality assurance and regulatory compliance through policy development, audits, and continuous monitoring of clinical processes. Ensures adherence to standards, reduces risks, and maintains high-quality care, patient safety, and organizational integrity.

Lead Financial Management

• Drives financial performance by optimizing resource utilization, managing referral networks, and collaborating with stakeholders to achieve cost savings and revenue growth across healthcare services.
