The Case Manager is responsible for assisting clients who are homeless to access housing and resources through assessment, case management, and advocacy with community partners. The case manager provides services according to grant guidelines. Contact may occur in non-traditional settings.



Essential Functions


  • Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services. May assist clients with application process for entitlement programs.
  • Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
  • Completes accurate and timely data entry into applicable data systems according to policies and procedures

  • Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies, including emergency and crisis services.
  • Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
  • As appropriate, ensures that clients have access to transportation to necessary appointments, and assists with client transportation according to the PHC transportation policy.
  • Uses professional discretion with client information while communicating with other agencies on behalf of clients, ensuring that appropriate releases of information are obtained.
  • Advocates for clients with community agencies, schools, significant others, and other appropriate agencies.

  • Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as a liaison between PHC and community resources.


Minimum Requirements


Education:
Bachelor’s degree in psychology, counseling, mental health, human services, or related field. Four years of experience in characteristic duties of position may be substituted for advanced education if allowed by granting agency.


Experience:
None.


License/Certifications:

  • Active Chauffer’s license from Iowa Department of Transportation.
  • Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health.
  • Must be obtained during introductory period if not current.


Knowledge, Skills or Abilities:


  • Excellent customer service skills.

  • Effective English communication skills, written and verbal.

  • Contributes to a team approach to care.

  • Proficiency with Microsoft Word & Excel and the ability to utilize database applications.

  • Demonstrates attention to detail in work.

  • Prioritizes assigned work.

  • Establishes and maintains effective working relationships with a diverse group of individuals.

  • Interacts with others in an empathetic manner while maintaining professional boundaries.

  • Remains calm and works through difficult interactions.

Education:

Experience:


  • Six months of work or volunteer experience in human services related activities. Experience acquired while obtaining a degree will be considered.

  • Work experience with vulnerable populations, including people living with HIV or those experiencing homelessness.


License/Certifications:
N/A


Knowledge, Skills & Abilities:


  • Demonstrated competency using HMIS or CareWare as applicable to department.

  • Working knowledge of community resources related to homelessness or HIV/AIDS in Des Moines.

  • Knowledge of program terminology.

  • Ability to effectively use HMIS or CareWares system to document all client contact.

  • Working knowledge of local community resources, chronic and acute mental illnesses, and chemical dependency.



Typical Work Hours


This position typically works 40 hours per week. The schedule for this position will be Monday through Friday, 8 AM to 5 PM.
