Summary:Responsible for assessing developing, and implementing appropriate social work care plans which take into account the psychosocial, financial, community resource, long term planning and bereavement needs of hospice patients and their families. Functions as a key member of the interdisciplinary healthcare team


Responsibilities


Responsibilities:Assesses the psychological, social and emotional factors related to the patient s illness, need for care, response to treatment and adjustment to care.Assesses risk factors or barriers to care and provides or refers appropriate services as indicated; facilitates contracts that outlines patient/family responsibilities in achieving the physician-directed treatment care plan.Assess the relationship of the patient s medical condition to the individuals home situation, financial and legal resources and availability of community resources.Crisis intervention to assure lack of abuse, neglect and exploitation of patient.Participate in interdisciplinary team conference and in team formation of initial patient care plan and updates of plan; provide consultation regarding psychosocial care.Advocates for the maintenance of individuals, families, agencies and the community resources and programs to support them.Provides short-term counseling to help patients/families adjust to their situations.Provide clinical inservices for staff; remains knowledgeable and assists in ongoing update of resources and programs available to assist patients in counties served.Documents patient care in compliance with organizational policy and regulatory standards.Participates in Quality audit and Quality management activities as assigned.Manages caseload within departmental productivity standards.Provides bereavement services to patients/families and community.Lead as the hospice volunteer coordinator to be responsible for the following: -Recruit and retain volunteers for Hospice. -Provide training, education and orientation to new Hospice volunteers. -Coordinating with HR to ensure volunteers are trained and receive appropriate background checks and health immunizations. -Hold monthly meetings for education and retention of current volunteers. -Facilitate assessment for volunteer placement in pt homes. -Coordinate volunteers in pt homes and assist with communication between volunteers and IDT members. -Community outreach (Fair) to educate community regarding the needs for Hospice volunteers and Hospice education.-Managing Desk File for Hospice volunteers in compliance with Hospice COPs. -Managing and recording hospice volunteer hours worked to reach 5%. -Monthly education to current Hospice volunteers to demonstrate competency. -Recruit and retain Volunteer Hospice Chaplains.-Notify volunteer hospice chaplain of new pt admissions. -Assist volunteer Hospice Chaplain with developing plan of care. Lead as the Bereavement Coordinator to be responsible for the following: -Bereavement assessment post mortem. -Develop bereavement plan of care based on assessment. -Bereavement visits and phone calls based on bereaved plan of care. -Supervising bereavement volunteer with bereavement mailings. -Assist with Epic build of bereavement. -Make referrals to outside sources of support for intensive bereavement counseling. -Assist with adjustment and transition to survivors of bereaved.*Performs other functions as required.


Qualifications


Other information:1. Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or2. Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by a MSW as noted above anda. Has one year of social work experience in a health care setting or b. Has a baccalaureate degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW.3.
