The Family Navigator works with families of individuals with disabilities and provides individualized plans for appropriate services. This position maintains knowledge of services offered within the organization and resources in the community and ensures plans meet the needs of the individuals. This position also represents the organization at events and provides training and consultations as needed.





Essential Functions
:



  • Provides families with individualized planning and guidance regarding services, including addressing barriers, developing goals, outlining steps needed to meet goals and connecting families to resources and services to support their needs.

  • Conducts follow-up activities with families and individuals to determine if ongoing support is needed.

  • Completes all documentation as required by the organization, funders, and accrediting bodies.

  • Represents the organization at conferences, resource fairs, and networking events.
  • Provides in-services, trainings and tours for health care providers, schools, and businesses/organizations as requested.

  • Provides master training and consultations with school districts and other agencies.

  • Assists with other services as needed, including training staff and social skills groups.

  • Able to manage crisis situations, provide immediate assistance and determine appropriate options for continued support.

  • Excellent written, verbal, and interpersonal communication skills to build relationships with a variety of stakeholders.

  • Excellent attention to detail with high-level organizational and multi-tasking skills that reflect the ability to perform and prioritize multiple tasks seamlessly.

  • Knowledge of computer systems and intermediate-level knowledge of Microsoft Office suite.

Qualifications:



  • Bachelor's degree in related field.

  • Knowledge of disabilities and understanding of services, eligibility requirements and funding sources.
  • Knowledge of Microsoft Office suite including intermediate-level knowledge of Word and Excel.




Additional Requirements:



  • Travels locally to meetings at organizations and agencies.

  • Must complete and maintain all required trainings/certifications.

  • Must maintain confidential information in accordance with HIPAA regulations.
