Job Title:  Coord, Patient Flow

Position Summary:  The Coord, Patient Flow is responsible for bed flow, management of admissions, transfers and discharges throughout the hospital.

Education: Current RN License, Graduate Accredited School of Nursing. BSN required.

Skills:  Critical thinking, decisive judgment and the ability to work with minimal supervision in a fast pace environment.

Experience:  minimum 5 years nursing experience in an acute care setting, previous experience in case management and insurance verification preferred.

Essential Functions of the Job:

  • Screens for the medical necessity of admissions and direct transfers to the Medical Center and the appropriate level of care requirements.

  • Prioritizes and assigns beds for all admissions and transfers based upon input from requestor, guidelines and input from receiving units based on their activity, staffing and bed availability.

  • Assures communication of bed assignments to the appropriate department

  • Coordinates and leads daily communication to facilitate bed placement and throughput.

  • Educates nurses, physicians and staff in level of care and placement issues

  • Screens admissions and evaluates clinical data using appropriate criteria to monitor for appropriate level of care orders and bed assignments

  • Maintains communication with patient access to keep informed and screening of transfers in and out of the facility.

  • Acts as a liaison between and with physicians, patients/families, hospital staff, patient care areas and outside agencies in an effort to promote efficient patient flow.

  • Escalates medical necessity issues, admission status issues and bed assignment conflicts.

  • Monitors and reviews the scheduled/elective reservation requests and prioritizes as needed working closely with affected services/units/OR when bed availability is limited.

  • Communicates with the Staffing Resource Office when resources are required to support patient needs. Works in collaboration with Directors, Nursing Staff, Managers, staffing coordinator, case managers and patient access to place patients

  • 12.Uses critical thinking skills as well as negotiating skills to prioritize patient placement based on clinical need.

  • Communicates effectively with ancillary departments to prepare patient rooms and expedite patient admission, transfers and discharges.

  • Able to appropriately delegate work to others to foster a team approach to timely patient placement.

  • Maintains accurate data requirements, statistical logs and prepares reports as needed.

  • 16.Participation in data collection for research or internal and internal/external benchmarking may be performed

  • Assists in insurance utilization reviews and discharge planning as requested.
