Providence is calling a Bereavement Services Coordinator (Days, 1.0 FTE) to Providence Hospice in Anchorage, AK. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will:

  • Coordinate the assessment, planning, and delivery of individualized and appropriate grief counseling services for families, caregivers, and hospice staff.

  • Plan for and implements Hospice Bereavement Program development, and expansion in response to industry and regulatory standards as well as identified patient, family, and community needs.

  • Monitor program activities for quality and excellence. Tracks and communicates program activity levels and outcomes to appropriate audiences (leadership staff, internal and external publics) as directed.

  • Participate in IDG discussions and care planning at both formal and informal team meetings. Document discussions and status updates in the Medical Record per Hospice policies and procedures.

  • Work collaboratively with the Hospice Volunteer Coordinator to recruit and place bereavement volunteers in appropriate service positions within hospice framework. Act as an on-going resource for bereavement volunteers by providing guidance, support, and developmental feedback where appropriate.

  • Provide continuing education to internal and external audiences on grief and bereavement topics.

  • Coordinate all internal and community events related to bereavement.

  • Maintain documentation of all assessments, care planning, and intervention, and customer feedback surveys for all hospice patient families.

Required qualifications for this position include:

  • 2 years of bereavement or grief counseling, chaplaincy, or social service experience that includes a focus on grief/loss counseling

  • 1 year of experience with program development

Preferred qualifications for this position include:

  • Bachelor's degree in related area

  • Automobile insurance according to Alaska State Motor Vehicle laws
